a) Go to FormReLeaf.com

b) Click in the right hand corner of your browser.

-Fill out the required information on the Account Owner Signup screen and Click

2) Logging In to FormReLeaf.com

a) On FormReLeaf.com, Click in the top right corner of your browser.

b) On the Log In screen, enter your account email and password and Click .
